Job Description

We are a stealth healthcare AI startup building the operations layer for
private medical clinic groups.
Medical clinics are drowning in repetitive administrative work: phone calls,
scheduling, referrals, billing questions, prior authorization, follow-up,
documentation, and handoffs across fragmented systems. These workflows are
painful for staff, frustrating for patients, and expensive for clinic owners.
We are starting with a signed pilot customer: a multi-location private medical
clinic group. Our first workflows are phone operations and prior
authorization. The goal is not to build a generic chatbot. The goal is to
build reliable workflow automation that works in the real world, with human
review, auditability, escalation, and careful privacy/security controls.
We are early, moving quickly, and looking for people who are excited by messy
operational problems. This is a chance to help build a vertical AI company
from the ground up.

About the role:
We are hiring a backend/integrations engineer to help build the workflow
infrastructure for our first clinic operations AI pilot.
This person will work under the founding engineer and help build the core
backend systems that support phone workflows, prior authorization workflows,
human review, auditability, and internal tools.
This is an execution role, not the primary architecture role. That said, we
need someone who writes clean code, communicates clearly, and understands that
reliability matters in healthcare operations.

What you will do:

* Build APIs, database models, workflow state, queues, retries, and background jobs.
* Implement internal tools, dashboards, status tracking, and admin workflows.
* Build audit trails and workflow history views.
* Build mock integrations and test harnesses for EHR/PMS and payer workflows.
* Support integration scaffolding for clinic systems, voice vendors, LLM providers, and workflow tools.
* Write tests and documentation for the systems you build.
* Work under senior code review and contribute to technical planning.
* Help make the system observable, debuggable, and safe to operate.
* Support QA and pilot launch readiness.

What we are looking for:

* 3 to 6 years of backend or full-stack engineering experience.
* Strong Python or TypeScript skills.
* Experience with relational databases such as Postgres.
* Experience building APIs and backend services.
* Experience with queues, background jobs, retries, or workflow state.
* Strong code hygiene and testing habits.
* Good written communication.
* Ability to work quickly under senior technical direction.
* Comfort with ambiguous, fast-moving startup environments.

Strong pluses:

* Healthcare, fintech, insurance, logistics, or other regulated workflow experience.
* Internal tools or admin dashboard experience.
* Experience with workflow engines, state machines, or process automation.
* Experience with Temporal, Celery, BullMQ, Airflow, Retool, or similar.
* Experience with auth, RBAC, audit trails, sensitive data, or security-sensitive systems.
* Experience working with AI-enabled workflow systems.
* Experience with EHR, PMS, RCM, billing, referrals, payer, or prior authorization workflows.

Role details:

* Location: Remote acceptable. US time-zone overlap required. Nearshore acceptable with strong communication.
* Engagement: Full-time contract.
* Compensation: Competitive and dependent on experience, location, and engagement structure.
