**About AtendaCare**
Millions of Americans live with complex chronic conditions β€” and millions more
care for them. Patients and caregivers alike are managing dementia,
Parkinson's, heart failure, and COPD at home every day, largely invisible to
the healthcare system meant to support them. That invisible care doesn't get
documented. It doesn't get billed. And the clinical teams managing these
patients are flying half-blind.

AtendaCare is changing that.

Our product, Atenda, is a voice-first AI companion for patients and caregivers
alike. Depending on the care setting and condition, Atenda meets the user
where they are β€” whether that's a family member navigating a loved one's
dementia or a patient managing their own chronic illness day to day. Atenda
listens, provides empathetic real-time support, and silently converts those
conversations into structured clinical summaries β€” delivered to the care team
as FHIR-ready data. We turn invisible care into documented, billable care,
using existing Medicare Remote Therapeutic Monitoring (RTM) and Chronic Care
Management (CCM) codes. No new legislation. No waiting.
Atenda launches in dementia care β€” a population with acute need, deep
caregiver burden, and strong Medicare reimbursement alignment. But the
platform is built to expand across chronic disease states, giving healthcare
organizations a single AI-powered tool for managing the full complexity of
their patient populations at home. One platform. Multiple solutions.

**Your Role**
We believe AtendaCare has the potential to become the operating system for
home-based chronic care. The founding engineer who joins us now will help
define the technology, culture, and future of the company from day one. _We
are looking for a true founding team member who will own the technical
architecture and implementation of Atenda from the ground up._

This is not a "senior engineer joining a team" role. You will be the technical
foundation of the company. You will make architectural decisions that will
live with us for years. You will work directly with the COO (who brings 20+
years of healthcare IT and solution architecture experience) to translate
clinical requirements into a compliant, scalable, production-grade system.

If you want to build something that genuinely matters, work at the frontier of
voice AI and healthcare, and have the space to do your best engineering β€” this
is the role.

**What You 'll Own**

* Voice AI pipeline β€” design and build the end-to-end conversational AI system: speech recognition, NLP/LLM orchestration, empathetic response generation, and text-to-speech. This is the heart of the product.
* Clinical data extraction β€” architect the pipeline that converts unstructured caregiver conversations into structured, FHIR-compliant clinical summaries ready for physician review and billing workflows.
* HIPAA-compliant infrastructure β€” design the cloud architecture (AWS preferred) with PHI-safe data handling, encryption at rest and in transit, audit logging, and a clear path to SOC 2 Type II.
* Mobile/web application β€” build the caregiver-facing app (likely React Native or Flutter) and the physician-facing dashboard where summaries surface.
* API and backend β€” design clean, documented APIs that can eventually support EHR integrations (Epic, Athena, etc.) and third-party clinical tools.
* Technical decision-making β€” select the stack, define the development standards, and set the quality bar. You will be the standard.
* Pilot readiness β€” get us to a real-PHI clinical pilot with a small physician practice. That is the near-term north star.

**What We 're Looking For**

Required:

* 5+ years of software engineering experience, with meaningful full-stack depth
* Hands-on experience building with LLMs and/or voice AI (OpenAI, Whisper, ElevenLabs, AssemblyAI, or equivalent) β€” not just API calls, but real product integration
* Strong backend fundamentals β€” REST APIs, databases, cloud infrastructure (AWS preferred), async systems
* Experience building mobile or web applications that real users depend on
* Comfort with ambiguity β€” you can ship something useful with incomplete requirements and iterate fast
* Strong written and verbal communication; you will work closely with clinicians and non-technical stakeholders

Strongly Preferred:

* Experience in healthcare IT, digital health, or any regulated software environment (HIPAA, HITRUST, FDA SaMD)
* Familiarity with FHIR/HL7 standards and clinical data structures
* Experience building for older-adult populations or accessibility-first design constraints
* Prior founding or very early-stage startup experience (employee #1–10)

**What We Offer**

* Equity: Founding engineer equity package (1–3%+, based on experience and fit) with a standard 4-year vest and 1-year cliff β€” meaningful ownership in a company we believe will be worth a great deal more than it is today
* Salary: $120,000/year to start, resetting to full market rate ($195,000+) upon the Company closing $4,000,000 or more in new equity financing or 18 months from start date, whichever comes first. This is a deliberate early-stage structure β€” the equity is priced accordingly.
* Remote: Fully remote, US-based, legally authorized to work in US without sponsorship
* Direct impact: You will work directly with the COO and clinical leadership β€” no layers, no bureaucracy, no ticket queues you didn't create
* Mission: We're solving a real problem for one of the most underserved populations in healthcare. The people who will benefit from this product are caregivers like our CEO, who spent five years caring for his wife through vascular dementia. This work is personal.

A note on compensation transparency: We are an early-stage company and we're
being straight with you about it. The initial salary reflects where we are
today; the milestone reset reflects where we are going. Both the salary
structure and the equity terms will be spelled out clearly in your offer
letter β€” no surprises.

**The Stack (What We 're Thinking β€” Open to Your Input)**
We are pre-stack at this stage by design. We want the right engineer to have
real input on technology choices. That said, our working assumptions:

* Voice/AI: OpenAI (Whisper + GPT-4o), ElevenLabs or PlayHT for TTS, LangChain or similar orchestration
* Backend: Python (FastAPI) or Node.js β€” strong opinions welcome
* Mobile: React Native or Flutter
* Cloud: AWS (HIPAA-eligible services β€” RDS, S3, Lambda, API Gateway, Cognito)
* Data: FHIR R4 for clinical outputs; PostgreSQL for application data
* DevOps: GitHub Actions, Terraform, containerized via Docker/ECS

**Our Regulatory Posture (What You Need to Know)**
AtendaCare is classified as a Class A SaMD under IEC 62304 β€” the lowest-risk
tier. We operate under FDA Enforcement Discretion (2023 guidance). Our
intended use is scoped to "informing clinical review only" β€” we make no
diagnosis and no treatment recommendations. We completed an independent
compliance audit with an A-minus rating.

You don't need to be a regulatory expert, but you do need to take HIPAA
seriously and build with compliance in mind from day one.
